MEDICAL RELEASE & MEDIA APPEARANCE WAIVERS REQUIRED FOR ATHLETES AND COACHING STAFF

The CHEERpros Medical Release & Appearance Waiver is now digital. During your online registration process, you will be required to enter your roster for each team, with a parent/legal guardian e-mail for each competitor, including all alternates. Once you complete your registration for the CHEERpros event, you will receive an e-mail confirmation, and every athlete’s parent/legal guardian listed on your roster (competitor & alternate) will receive an e-mail link for the Medical Release & Media Appearance Waiver for their athlete to complete. If you need to add, delete, or change your roster, please contact Mai Lewis at mai@cheerpros.com, and she will be able to assist you with the updates.

Coaching Staff (i.e. Advisors, Directors, Coaches) are also now required to have a completed and signed waiver. Make sure you set-up your coaching staff in Themis and attach them to the appropriate team(s) - Email addresses required so that the waiver is sent directly to your staff.

All athletes must have a signed online waiver completed to participate in any CHEERpros event. Waivers are not transferable from event to event. A new waiver will be required for every athlete and coach for each CHEERpros event. Thank you for your understanding and cooperation.
